Curious if anyone has ever seen this done before and can offer some insight why someone would do it.

Some history...Seemed to have a leak somewhere and found this when I removed the liner from the bed.

Is that the front panel of the bed ? close to the cab ?

drug vehicles cut out the sheet metal in the bottom of the bed
coyotes smuggle people buy cutting out sheet metal in similar ways

other ingenious souls may try to make a pass through from the cab to the bed & put a cab over camper on it, like a motor home

I've seen similar many years ago... it was a little less nefarious than what has been suggested above.

Of course they had also removed the entire rear window of the truck, and then sealed the "pass through" to the camper shell with some sort of gasket material..

Well that is not as exciting as the previous answers but also an option.

Did the truck have any aftermarket audio wiring in it when you got it?

Plain and simple that is a hole for a CB Radio Antenna

I have seen a couple of trucks with pass throughs like that for the sub boxes that where in the front of the box.
