exhaust leak 02 sensor

had a trouble code for my rear heated o2 sensor too high voltage. went to check it, and saw a huge hole in my exhaust pipe just before the problem sensor (between cat and sensor) I patched it up...

alright, voltage too high. I also noticed a golf ball sized hole in my exhaust just after the cat and just before the o2 sensor...could this cause anything?

p0138 help!

Ok so I failed emissions, terrible gas mileage, cel on, and a p0138 trouble code. Its obvious I have a bad o2 sensor. It says bank 1 sensor 2.
