They just like the 5-bolt, it's been the Ram pattern since '97. there's been no real reason to change it since Chrysler doesn't cater to the aftermarket in any way.

Because manual transmissions are not a selling feature on trucks these days. Aside from the performance crowd, which is slim-to-none in the truck market, there's not much to be gained with a manual over an automatic. Having 1 trans also allows them to get specific EPA ratings and makes the design and manufacture a hell of a lot simpler.
