Default Injector Tapping?-05 5.9L

My girlfriend had her 05 2500 inspected and oil changed today. On the way home I noticed there was a tapping that I hadnt heard before and blowing black smoke around 1500 rpm with normal acceleration/driving. Powertrain wise the truck is stock and will usually will on blow smoke when you really get on it.
I checked the oil which was good and listened to each of the cylinders and it seemed loudest at the fourth cylinder.
It only taps when the truck is at running temperatures and is gettingfuel, at the same rpm down hill its gone.
Could this possibly just be from bad fuel or should I be expecting to replace the injectors soon? Not even the injector at all?
