We ran a police-based ALS system. The police would respond as paramedics to medicals while the firefighters would respond in a BLS capacity with the ambulance. Our police-medics carried oxygen, a full bag of medical gear including narcs, and a full heart monitor. It was a great system. In the middle of the night when us fire guys would have to get up, get dressed, get to the station, map it out, wait for a crew, and the respond, the police were already on-scene providing ALS level care. Police-medics made 7% more than a regular RPO.

We are slowly moving away from the model...staffing and finding qualified people is very hard to manage. EMS is becoming completely fire based.

We currently run around 2K EMS runs a year and around 500 fire calls (ranging from a smell of gas to structure fires).

It's enough to keep me busy on my days off...

My full-time job I am a dispatcher for Hennepin EMS based out of Hennepin County Medical Center in downtown Minneapolis. 65,000-70,000 EMS runs a year. We just became the largest EMS service in the world to install Stryker's new Power Load system in all of our trucks. Very cool system. A lot of backs will be saved with this system.
Sounds like a very good system y'all got there.

The Stryker system is very good. I've worked with it many times in the hospital.

I loved it. We were only one of three agencies in the state of Minnesota that ran police-medics. Maplewood MN got away from it years ago (same issues we are having), and the city directly north of mine, Woodbury MN, takes it one step further and requires all police officers to either be a paramedic or firefighter.

Yeah, being "just" a dispatcher, I haven't gotten to use it as much as I would have liked, but the medics just rave about it. Going through the training was a real eye opener as to how well it's engineered. Amazing staff they have.

Pretty much everybody here in California runs both. Whether or not fire runs ambulances or just engine response varies though. When I was in SoCal, most of us ran our own ambulances, but up here it seems more common for the ambulance service to be a private like AMR. Most of our paid fire agencies are ALS and the volunteer agencies are BLS, though one of those was working on a limited scope ALS program with the county.

My department here was BLS when I first started, and in 2011 we started an ALS program with at least one medic on the engine 24/7. We deal with a lot of traumas and drug/alcohol related calls (university campus) so having medics on the engine has made a big difference.

we need an "other" option. Our EMS & fire departments share a building. You can be on just one or the other or both so we are kind of together but not.

We do Fire/Rescue and QRS (Ems) out of our station. Our calls are about half Fire/Rescue and other half EMS calls.

I am retired now.
I was a firefighter/paramedic, weran rescue trucks/no ambulances
I was a city firefighter/paramedic,ambulance duty was provided by the county.
the county wouldn't abide by our dailey policies,so we wouldn't let them bring their personnel inside our firehouses.
we worked 24/48 shifts, most ambulances around here were 8hr. shifts
