sounds to me like it could be a linkage problem. also when rootbeer said something about going to the side (right) to engage 4low but your manual said nothing about going to the side you probably have a different model trasfer case than he does. To my knowledge all the half ton rams came with an np231 transfer case (yeah the same one in most TJs and z71s). however you have multiple models of that transfer case. My '94 shifts to the right for 4lo and my dad's '01 is all the way forward. But the grinding noise you hear and the fact that its not engaging correctly could be that the shift linkage itself is bent or out of adjustment. it can be adjusted by moving a block with a set screw on the linkage on the side of the case (much like adjusting the transmission shift linkage). I grounded mine on a log one day and had the exact same symptoms you're having. it took me five minutes with a rachet and my four wheel worked perfectly.

I have found that on my 99 1500 5.2 when i switch into 4wd hi it seems to be easier when the truck is off. I've only used 4wd hi a couple of times though. Taking it out of 4wd hi to low, in neutral or park, truck wheels pointed straight, truck fairly straight no problems going out. Sometimes i've noticed coming out of 4wd into 2wd it takes a few seconds of driving for it to actually come out of 4wd

my 98 should be the same thats when they came out with the on the fly shifters, my truck ive figured out alll the tricks to and im sure its the same with allt the others. you cant be going faster than 20 mph when engaging 4H, when engaging 4H you should feel or heer a clunk, not a nasty clunk just the 4WD engaging, with my truck when i put it in 4H i usually sit still, engage it, then i put in in reverse then drive and it allways engages that way. then 4WD light should turn on. now, when engaging 4Low, i try to explain this all the time lol, you need to be rolling but the truck has to be in neutral so put the truck in drive let the truck roll, and while the truck is rolling reach down and put the shifter into 4Low (still rolling) and it WILL go in lol. thats how it is with mine, and a couple other buddy's trucks with dodges.
