get a hammer knock the old ones out, make sure the drum isnt bored out from the wheel being loose if not just knock the new ones in until they seat

If you find the drum is a bit difficult to remove, use a screwdriver on the shoe adjustment wheel. It's on the back side of the brake assembly, there is a slot you can use the screwdriver to spin it. It looks like a gear, has teeth that the screwdriver will fit in.

I know this because I too have had a rear wheel come off on my 85 Ram

I prefer to use a lug nut to seat the stud, draws it in nice an tight, and you don't have to worry about not having it exactly lined up and damaging the drum.
