Default Irregular RPM at low speeds

2005 Ram Hemi w/ 88000 miles.
Starts up and idles is normal.
Once I start driving, during acceleration and at steady low speeds the rpm randomly drops around 300-400 rpm and then jumps back up again in rapid succession making the truck jump/jerk a lot.
Doesn't happen once I reach highway speeds and it seems to get better once the truck warms up.
No codes on the built in key activated code reader or from the plug in reader.
Yesterday I ran in to an additional problem, while test driving about 15 mph and it started to jump and jerk real bad because of the rpm fluctuating so much, the engine simply shut off. It started back up without any problems, but caused me some concerns that it might happen at higher speeds.
I cleaned out the throttle body and TPS, but that didn't help.
Any ideas on what could be causing this ???

Fuel pump? Just throwing out a guess.
