Mine hardly ever comes on. Only when I'm holding steady at 70-80 km/h (40-50mph) on fairly level ground, and not too windy. Bit of lift, aggressive tires, and carrying weight contribute to that.

The "Fuel Saver" light on an 2009 does not indicate the truck is in MDS mode.
All it means is that you are driving in a fuel efficient manner and that MDS may be active at that time if all other criteria have been met.

It is true that MDS will only become active in "Fuel Saver" mode.

"Fuel Saver" and MDS are two different functions.

'09 Fuel Saver light as mentioned ONLY indicates that the computer has determined that you are driving in a fuel efficient manner
Fuel Saver & ECO mode are the same thing, just used in different year models

On my '09 i can have the light on & the engine sounds normal, as i slowly build the speed up to 28 mph, the sound changes to more of a idling motor boat sound, like farting in a pool. People on the sidewalk start looking around to see where the noise is coming from.
At that point i downshift to get it out of over drive, which kicks it out of MDS mode

You sure about that?
I know the ECO mode is independent of MDS, with my exhaust the rare times I see the fuel saver light it coincides with the horrible sounding buzz

Mine only makes the fart in the pool sound when the fuel saver light is on . As soon as she stops farting the light goes off on visa versa . Never has one symptom appeared without the other . I'm pretty sure there connected

09 has no "ECO" mode, only the fuel saver light.
They are considered to be the same, however the fuel saver light only coincides with MDS
Eco mode does not necessarily coincide with MDS
As far as I can tell..

That's what I was thinking, I just wanted to be sure I was on the same page since I haven't driven my dad's '09 in a while.
