Default shifting problem

My son has a 1996 model that is intermittantly not shifting properly. At takeoff, it shifts and gains speed as it is supposed to, but it will not shift into overdrive and does about 50 mph at a little under 3000 rpm. It does it sometimes and sometimes it doesn't, with no common denominator. I was wondering if there is a common problem that would be a good start point. I don't know if there is a solenoid, valve, or switch that might be stuck, causing it to do this. Obviously, it is a 5.2L, automatic tranny, with about 120,000 miles. He had the tranny fluid changed and tranny flushed about a month or so before it started doing this.
