Default 06 mega cab suspension sag, question

ok so i have a 06 ram 2500 mega cab, has the hemi... it is lifted 12" on 41's
(yes i know, lol) anyways i just bought the truck and it has a noticable lean about 1/2inch or so on the drivers side and is consistant front to back. any ideas what could cause this, i looked at everything and nothing is bent or tweaked that i notice, just seems weird that both front and back on on side know what i mean.... so any ideas guys??

what about with a full tank of fuel, think that would affect it and if it would the whole side lean a bit?
