Default Surging when going into OD/TQ lockup...I think (long but detailed)

My truck has a slight low idle issue. Not a big deal, kinda intermittant, and not really causing any problem yet. Gonna try a new TPS and/or IACV when I have time. I also have a strange surging issue while accelerating that I at first thought was likely related to the idle issue but now I'm thinking maybe not.

It is difficult to explain the surging issue but it seems to only happen when the truck is at a low RPM and low throttle. It doesn't seem to do it on take off at all....it runs out smooth and normal. It also doesn't do it at freeway speeds....whether running 70-80 on flat road or even the times when I hit a hill and it starts to bog and has to drop a gear, it never does it. Those times that it runs smooth as can be are what got me to thinking it may not be related to the idle issue. It seems like it only does it in the higher gears at relatively low speeds, usually when I am still accelerating. It seems to be more noticeable when the truck is still in a higher gear and under a light load, like climbing a hill. It's almost like a light surging or bucking that you can feel and hear in the exhaust. It may even only do it when the TQ is locked/and or the truck is in OD but I'm not certain. I do know if it is doing it and you stomp on down on the pedal, it will drop a gear and go on smooth as you please.

I have read about TQ surge and I don't really think that is what I am feeling. From what I read, TQ surge is very brief and almost like a vibration. My issue is more drawn out and can last a couple of seconds or maybe longer under the right circumstances. I also read that TQ surge happens usually just prior to lockup. Whatever this is seems to happen right after the tranny goes into a higher gear. The truck changes into a higher gear (or maybe it's going into lockup or OD...not sure), the RPMs drop, then it immediately starts this surging that gets less noticeable as the truck gains speed and RPMs climb back up. This all normally takes 3-5 seconds to get back up to a speed/RPM where the surging just fades away.

So a few questions that might help me diagnose it and help me understand it better. Can someone explain TQ lockup and OD as applies to our trucks? I know what a TQ is and what lockup is but how does it work in our trucks? What decides when it locks/unlocks? Can it lock in any gear under the right circumstances or does it have to be in a certain gear or higher? Also, how does OD work in our trucks? I know the tranny is a 5-speed auto. Does that mean the tranny changes out four times when starting from a stop and accelerating to cruising speed? Is the top (5th) gear considered OD? I feel like answers to these questions will better help me determine what my truck is doing.
