what is best mods for save MPG with my 5.7 HEMI ?

cold air intake
dual exaust
power programmer

Since our '11 and '12 trucks don't have a programmer yet, just go with the CAI and exhaust. You used the term dual exhaust, but it seems you get just about as much bang-for-the-buck with a simple, higher efficiency muffler swap...unless you're going full headers and all. That's what I did with my '12 Express, and I love it.

for exhaust have you ever compared different option? What gives the best return for money

the original OEM single line with a good muffler and remove the resonator.

the double line OEM as the sport model with a good muffler.

real double line with a two catalytic converter with a good muffler.

when i worked in a dyno shop different engines react differently to each style. to say which exhaust option works best would be rather expensive. but best for the money would be in order

with that said yes a programmer isnt an option right now.but with the exhaust a full exhaust headers back is the best but really expensive with minor results. so a simple muffler swap would work best. saves wieght and frees up exhaust pressure. but you must be careful to much flow will yield negative results. unless built for it most motors require a certain amount of back pressure.
intakes also provide different results.there are 3 types stock,short air, and cold air. they all do different things.

I've just got the programmer and muffler w/o cats now. Next is going to be LT headers and off-road Y. I'm thinking about ditching the spare tire to remove some weight, and make the truck a tad lighter=more mpg.

removing the spare is a start but below are some other options to consider :)
Taken from NA. Bold is what i have done.
1 lighter tires and wheels = can save you 100lbs - Done
2 remove tail gate = can save 69lbs to 110lbs - Done
3 remove spare tire and jack pieces can save 71lbs - Done
4 remove most gas in the gas tank [1/4 tank or less] can save 100lbs to 165lbs
5 remove window washer tank with water = can save 9lbs - Done
6 remove wipers = can save 1 1/2lbs
7 remove stock air intake system. = can save 2lbs - Done
8 run light weight pullys and short belt. save 1-2lbs - Done
9 remove fan and shroud with elec. fan = can save up to 10lbs - Done
10 remove passanger seat and cener seat = can save 80lbs - Done
11 lighter battery = can save 12lbs -
12 shorter exhaust system = can save 12lbs
13 remove any extra water from the radiator tank = can save 1-2lbs
14 remove sound system = can save 4lbs to 100lbs
15 remove sound proof material and carpet = can save up to 60lbs - Done
16 removed the front sway bar = can save 20lbs - Done
17 remove one leaf from each side. = can save up 25lbs - Done
18 remove A/C compressor, condenser, lines can save up to 74lbs - Done
19 remove hood liner = can save up to 3lbs
20 remove stock plastic wiper cowl = can save 3lbs
21 remove factory reciever hitch and bumper = can save up to 100 lbs - Done
22 remove all 4 fender wells = can save up to 15 lbs

add all these up and you got an average somewehere in the neighborhood of 8-900lbs!

Are you building a race truck? Otherwise removing all that stuff is stupid. If you were worried about saving on gas, get rid of the truck and get a Prius

I noticed very little change in mpg until I added the third piece to my set-up. First was the intake (spectre), louder and no mentionable change in power or fuel economy. Second was the Programmer, decent bump in power and very minimal change in fuel economy. Worth the money spent. Third was the exhaust. I replaced the muffler with an 18" Magnaflow and removed the resonator (3" tailpipe). Once again a decent bump in power and noticable mpg increase. I think once I added the exhaust it allowed the intake to reach it's potential, and same for the programmer. They all work together and compliment one another. Ive seen an increase of a solid 8% around town and still figuring out the highway mpg (I dont drive my highway). All in all 8-10% increase.
I would start with replacing the muffler and removing the resonator. Keep the piping you already have. Then find a good deal in a CAI.
