Engine pulsating??

Are rpm's actually changing or is it just a sound?

It appears it’s just sounds like a pulsating. I don’t really see a major change in rpm’s but I can feel a slight sluggish reaction in the truck’s movement. If that makes any sense. Thanks for replying.
